Recent and Long-Term Soccer Heading Exposure Is Differentially Associated With Neuropsychological Function in Amateur Players

Objectives: The present study examined the relative contribution of recent or long-term heading to neuropsychological function in amateur adult soccer players. Participants and Methods: Soccer players completed a baseline questionnaire (HeadCount-12m) to ascertain heading during the prior 12 months (long-term heading, LTH) and an online questionnaire (HeadCount-2w) every 3 months to ascertain heading during the prior 2 weeks (recent heading, RH). Cogstate, a battery of six neuropsychological tests, was administered to assess neuropsychological function. Generalized estimating equations were used to test if LTH or RH was associated with neuropsychological function while accounting for the role of recognized concussion. Results: A total of 311 soccer players completed 630 HeadCount-2w. Participants had an average age of 26 years. Participants headed the ball a median of 611 times/year (mean=1,384.03) and 9.50 times/2 weeks (mean=34.17). High levels of RH were significantly associated with reduced performance on a task of psychomotor speed (p=.02), while high levels of LTH were significantly associated with poorer performance on tasks of verbal learning (p=.03) and verbal memory (p=.04). Significantly better attention (p=.02) was detectable at moderately high levels of RH, but not at the highest level of RH. One hundred and seven (34.4%) participants reported a lifetime history of concussion, but this was not related to neuropsychological function and did not modify the association of RH or LTH with neuropsychological function. Conclusion: High levels of both RH and LTH were associated with poorer neuropsychological function, but on different domains. The clinical manifestations following repetitive exposure to heading could change with chronicity of exposure. (JINS, 2018, 24, 147–155)

A myriad of specialized and fragmented global regulatory bodies wield ever-increasing power and influence. In making decisions, these mission-oriented authorities tend systematically, due to deep-seated structural factors, to give greater regard to the interests and concerns of some actors, especially powerful states and well-organized economic actors, and lesser regard to the often peripheral interests and concerns of more weakly organized and less powerful groups and of vulnerable individuals. The overall pattern of global regulation reflects a similar bias. The most powerful global regulatory regimes promote the objectives of dominant states and economic actors, whereas regimes to protect weaker groups and individuals are often less effective or virtually nonexistent and are thus unable to protect their interests and concerns. As a result of these two types of disregard, the dominant actors in global regulatory governance enjoy disproportionate benefits from international cooperation, while weaker groups and individuals suffer deprivation and often serious harm.

Summary
Migraine headache is an extremely common and temporarily disabling headache disorder. Most studies on migraine prevalence have reported variation by age and gender. Prevalence is generally highest between the ages of 25 and 55, often with a peak in the late 30s and early 40s. Migraine was once considered to be a disease of the affluent. The American Migraine Study showed that migraine prevalence was lower in African-Americans than Asians. Familial aggregation of migraine has long been recognized and genetic studies have generally supported a role for both genetic and environmental risk factors in the etiology of migraine. The heterogeneity of familial hemiplegic migraine (FHM) underscores the likely heterogeneity of the more common types of migraine. Diagnosis of migraine is complicated by the episodic and heterogeneous nature of the illness. Population-based studies demonstrate that migraine is comorbid with depression, anxiety disorders, and manic depressive illness as well as epilepsy.

Summary
This book provides a comparative analysis of environmental regulation in multi-jurisdictional legal and political systems, focusing on three important examples: the United States, the European Union, and the international community. These systems are, of course, quite different in many ways, political, institutional, legal, cultural, and economic. Yet they share certain fundamental structural similarities and must deal with similar problems in instituting measures to protect the environment while simultaneously promoting economic growth. This book develops a linked series of conceptual frameworks for understanding and evaluating the ways in which these three multi-jurisdictional systems are dealing with this challenge, drawing on the insights afforded by comparative analysis of their similarities and differences.
Each of these systems must deal with environmental interdependencies that transcend their constituent jurisdictions – states in the United States, the Member States in the European Union, and the nations of the international community. Pollution often crosses local borders, in some cases creating regional problems, such as acid deposition, ozone-type smog, and pollution of shared water bodies. Other transjurisdictional environmental problems are global, including stratospheric ozone depletion and climate change. Wastes are shipped across borders. There is a global stake in preservation of biodiversity, regardless of the jurisdiction where it is located. Yet, despite the maxim, held by certain ecologists and environmentalists, that everything is connected to everything else, some environmental problems, such as municipal waste disposal, many forms of pollution and resource development, and drinking water quality, remain localized in their effects on health and the environment.

Some transjurisdictional environmental problems are global, including stratospheric ozone depletion, climate change and the loss of biodiversity. Other environmental problems, however, are localized in their effect on health and the environment: for example, municipal waste disposal, many forms of pollution and resource development, and drinking water quality. These varying jurisdictional and environmental circumstances pose the central question of how responsibility for addressing different environmental problems should be allocated among the different levels of decision making and implementation in a multi-jurisdictional system.

Introduction
The failings of state socialism in the former Soviet Union, Eastern Europe, and many developing countries have stimulated renewed appreciation of the economic and political virtues of competitive markets in harnessing the efforts of managers to the demands of consumers, promoting efficiency in resource allocation, stimulating innovation, and avoiding undue concentration of political economic power. Many nations have taken steps during the past twenty years to dismantle central planning, privatize state-owned enterprises, and reduce or transform government control of economic activity.
Yet, during the same two decades governments have adopted and continually expanded far-reaching centrally planned command-and-control regulatory programs in order to limit air and water pollution, deal with toxic wastes, and solve other environmental problems. Seemingly oblivious to the inherent and well-documented failures of central planning, these programs aim to produce environmental quality by issuing detailed orders to thousands of individual facilities, prescribing their conduct in labyrinthine detail. This chapter seeks to help resolve this paradox by analyzing the advantages of economic incentive systems (EIS) for environmental protection including environmental taxes and tradable pollution quotas, identifying the types of environmental problems for which they are best suited, diagnosing structural factors that impede wider use of such instruments, and proposing regulatory reform strategies to promote such use.
The traditional instrument of choice for all types of economic and social regulation has been command and control.

ONE YEAR FOLLOW-UP OF COGNITIVE THERAPY, ANALYTIC PSYCHOTHERAPY AND ANXIETY MANAGEMENT TRAINING FOR GENERALIZED ANXIETY DISORDER: SYMPTOM CHANGE, MEDICATION USAGE AND ATTITUDES TO TREATMENT

A one year follow-up is reported of a randomized clinical trial with generalized anxiety disorder (GAD) in which the main comparison was between analytic psychotherapy (AP) and cognitive therapy (CT), each delivered at weekly or fortnightly intervals over a six month period. CT was found to be significantly more effective than AP. However, GAD is a chronic and relapsing condition and follow-up data are needed to assess the durability of improvement and the possibility that the benefits of analytic psychotherapy may be more apparent over the longer term. Data were collected in three areas: (1) symptomatology and overall improvement; (2) medication usage and contact with GPs; and (3) attitudes to therapy. CT was clearly superior to AP on the main outcome measures and only a minority of AP patients made significant improvements. CT but not AP was associated with significant reductions in medication usage, and patients receiving CT were generally more positive about treatment received. Differences between treatments were less evident in the less intensive treatment condition where overall results were relatively poor. The most positive outcomes were achieved in the more intensive CT condition in which approximately two-thirds of patients achieved clinically significant improvements.
